Under-eye circles treatment by lipostructure or lipofilling
Under-eye circles are cavities situated under the lower eyelid near the orbital edge.
They can extend inside towards the root of the nose and outside towards the cheekbone.
Their appearance varies from person to person. Circles give a tired look and are the first sign of aging in a social context.
There are two types of under-eye circles:
- deep circles: they may be associated with under-eye puffiness and are one of the most visible signs of aging. They may also be associated with a volume loss in the cheeks.
- dark circles: often old because genetically based, the only treatment existing is based on aesthetic medicine.
Treatment will vary according to the appearance of the under-eye circles.
Under-eye circles treatment by lipostructure or lipofilling
Under-eye circle lipostructure (lipofilling) is a surgery aiming at filling the under-eye circles by injecting the patient's fat directly in the deep circles for an excellent result.
The cavity between the cheek and the eyelid is filled and the look becomes harmonious again and seems less tired.
Lipostructure is also used to resculpt some sagging areas of the face such as the cheeks, the temples or the eyebrows.
Before the procedure
Under-eye circles lipostructure is a logical and physiological technique which recreates the fat tissues normally present along the orbital bone. The melting of these tissues creates the under-eye circles.
A thorough clinical and photographic study of the necessary corrections will be done.
In case of lipostructure and facial rejuvenation, the modalities of ageing need to be evaluated by comparing photographies of the youth of the patient and his/her actual state.
A preoperative check-up is made following your surgeon’s recommendations.
An anaesthesiologist will see you in consultation at the latest 48 hours before the operation.
No aspirin-based medication should be taken during the 10 days preceding the operation.
Under-eye circles lipofilling is a cosmetic surgery, it is not paid for by health insurance.
The procedure
The fat injected in the under-eye circle is usually taken from the abdomen. Fatty bags can be removed during an associated Blepharoplasty of the lower eyelid (bags under the eyes, on top of the deep circle).
The extracted fat is centrifuged in a sterile way.
The blood and the oil extracted during the centrifugation are disposed of, only the quality solid part of the fat is used for under-eye circles lipostructure.
The procedure always takes place in a sterile operating room and a short hospitalization is needed (one day in a clinic).
The fat is injected with a 1 mm cannula in contact with the bone and in depth, so it cannot be visible by transparency under the skin.
Indeed, the risk lies in the appearance of little points of a few millimetres corresponding to the fat injected too shallow or in excessive quantities.
In rare cases where the excess of fat appears, it can be removed surgically.
Most often, the anaesthesia is local (truncular anaesthesia) and associated with sedation given by intravenous drip.
The result is immediate, but the final aspect will not be visible before 3 months.
After the procedure
The result is final.
30% max. of the injected fat resorbs in the first 3 months and the remaining injected fat is definitely integrated. This is the reason why a second lipostructure session must be discussed during the consultation.
Under-eye circles lipostructure is adapted to every patient with deep circles.
Lipostructure is a genuine surgical procedure and we believe it must be used, preferably, when several areas of the face need to be treated:
- Associated with under-eye bags treatment
- Associated with an extended lipostructure of the face: cheekbones, eyebrows, lips...
- Associated with a face rejuvenation or a look rejuvenation surgery

Before the procedure
- Medical exams: 1 or 2 surgical consultations, 1 consultation with the anaesthesiologist.
- Recommendations: stop smoking 1 month before, stop aspirin-based and anti-inflammatory medication 10 days before the surgery.
- Signing of the estimate, informed consent and medical photographies.
- Reflection period: 15 days between the first consultation and the surgery in case of a cosmetic procedure.
The procedure
- Anaesthesia: local, neuroleptic or general (systematic if the patient is a child).
- Duration of the procedure: 1h.
- Return home: the very same day in most cases.
Follow-up and results
- Special care: none.
- Removal of the sutures: on the 5th day Stitches are not systematic, a steristrip can be sufficient.
- Desocialisation after the surgery: 7 to 10 days.
- Physical activities: 3 weeks.
- Professional activities: 7 to 14 days after the surgery, depending on your professional situation.
- Final result: 6 months.
